Primary Open Angle Glaucoma

Disease diagnosis:

1. TCM diagnosis

(1) Eye bilges, dizziness and headache, left and right migraine, bilge pain in brow bone, forehead and eye socket, and blurred vision.

(2) IOP increases at the early stage of the lesion and gradually increases with the development of the lesion; the peak and fluctuating values of IOP can be found after 24 hours of intraocular pressure measurement.

(3) Fundus change: the physiological depression of optic disc gradually deepens and enlarges, the cup/disc enlarges(C/D > 0.6), the color of optic papilla becomes pale or pale, the blood vessels shift to the nasal side or become geniculate.

(4) Perimetry: paracentral scotoma, arcuate scotoma, nasal ladder, late tubular visual field.

2. Diagnostic criteria of western medicine:

(1) Abnormal intraocular pressure: pathological intraocular pressure increases (IOP > 21mmHg in at least one eye) or normal (IOP ≤ 21mmHg), IOP fluctuates more than 8mmHg in 24 hours, or IOP difference in both eyes is more than 5mmHg.

(2) optic disc damage: optic disc depression deepens and enlarges progressively, disc edge becomes narrow, notching, optic disc hemorrhage, asymmetry of optic disc, and retinal nerve fiber layer defects.

(3) Anterior chamber angle examination: the anterior chamber angle is open, sometimes more iris processes (comb ligaments) can be seen, iris root adheres to the anterior side, trabecular meshwork pigment more and so on.

(4) Visual field defect: paracentral scotoma, arcuate scotoma, nasal ladder, and late tubular visual field.

Auxiliary examination as necessary: ERG, VEP, HRT, UBM, OCT.

Characteristics of Traditional Chinese Medicine Treatment:

1. Acupuncture treatment

Acupuncture syndrome differentiation treatment can effectively relieve the symptoms of glaucoma patients such as eye bilges and headache. For glaucomatous optic atrophy patients, long-term acupuncture can improve visual field and stabilize intraocular pressure to a certain extent.

2. TCM Syndrome Differentiation Treatment

The TCM ophthalmology divides the  bluish glaucoma (primary open angle glaucoma) into four basic syndromes, including: Qi stagnation syndrome, syndrome of spleen deficiency and dampness, deficiency of liver and kidney syndrome, deficiency of both Qi and Yin syndrome. Patients should select the medicine according to the type of syndrome.

3. Acupoint application therapy

Acupoint application around the eyes can effectively relieve the symptoms of open angle glaucoma patients, such as eye bilges and soreness. Most glaucoma patients with long-term use of intraocular pressure lowering drugs are more likely to have dry eyes. Through point application treatment, discomfort symptoms for glaucoma patients, such as dry eyes, can be effectively improved.

Western medicine treatment:

It includes local intraocular pressure reduction, neurotrophic and fundus microcirculation improvement drugs, laser and surgical treatment.
