Technique of Puncturing into Quchi and Binao with Six Cun Gold Needle

Using a 6 cun needle, made of gold, to cure lymph nodes (TCM called "scrofula") is a original treatment of Professor Wang Leting in our hospital. Scrofula is mostly related to liver qi stagnation and multiple abscess caused by damp-phlegm, most of which will lead to deficiency of lung-qi, loss of spleen movement, yin deficiency with deficiency fire, showing deficiency of both qi and yin. Professor Wang takes the Yangming Large Intestine Meridian of Hand as the primary site of treatment, which is the source of qi and blood. Specifically, it means puncturing into Quchi opposite to affecting side, then inserting horizontally towards Binao. Relevant records can be found from some classic medical books, for example, Leijing Tuyi recorded that "puncturing into Quchi … is mainly used to treat scrofula, pharyngitis and temporary inability to speak …"; "inserting towards Binao is used to cure arms' pain and weakness, fever scrofula and acute neck pain"; "acupuncturing Wuli is mainly used to treat ...fever scrofula". Acupuncture for A Hundred Diseases in Verse (Bai Zheng Fu) said: "puncturing into Binao and Wuli is used to treat scrofula". According to the above records, we know that the treatment of scrofula by acupuncturing Quchi, Wuli and Binao has certain theoretical basis. What's more, when the method of puncturing is used, scrofula can be better treated by stimulating Quchi, Wuli and Binao at the same time. According to Wang's experience, gold, silver, copper and iron can all be used to make needles, but gold needles are the best. The gold needle used by Wang leting is his own invention, which is forged with 90% gold and 10% brass alloy. The gold needle does not change with the cold and hot weather in four seasons, and is suitable for human body temperature. The patient's pain is not obvious when the needle is inserted quickly. The needle will not be out of shape, and there are no side effects. The needle hole is not easy to become inflamed and the therapeutic effect is very good. The technique of six cun gold needle puncturing has been applied to lymphatic diseases including scrofula, amygdalitis, mumps and some immune diseases in our department and good clinical results have been achieved.

The operation method of six cun gold needle puncturing into Quchi and Binao is as follows: before the operation, it is necessary to carefully explain the matters needing attention to the patient, so as to achieve better cooperation. When performing the operation, the patient is asked to sit with hands crossed on shoulders. The doctor first locates the Quchi acupoint with his fingers, and then disinfects the area around the acupuncture point with 75% alcohol. Next, the doctor takes a six cun gold needle and applies a little sterilized paraffin oil to the needle tip. Then the doctor's one hand raises the patient's elbow joint to shoulder level, the other hand holds the needle handle, and inserts the needle tip at Quchi. The needle tip is quickly inserted the skin at a centripetal angle of 45 °. Then the needle body is placed flat and slowly puncturing into Binao and only the handle is left out of the skin. In the process of inserting, some patients would feel distending pain. After inserting, the corresponding operation should be performed according to different conditions. The needle is usually kept in the body for an hour after it is inserted. During this period, the patient should keep his hands crossed on his chest and stay in a quiet environment. He should not talk to people and carefully experience the acupuncture feeling. Finally, pick up the needle slowly. If there is obstruction, the needle would be twisted until the obstruction is gone. Once the needle is out, press the acupuncture hole with a sterile, dry cotton ball.
